I have posted the following to the WP Plug-in page.
Love the SLP plug-in. You load it and it just works. Should you need support with the product, Cici is there to help. Not only does she provide you with a solution to your issue but she also takes the time to educate you on troubleshooting. I would recommend this plug-in to others.

There were a couple of issues going on at the same time. The theme I was using, Cacoon, has not been updated in several years plus a plug-in, WP Super Cache, appeared to be causing trouble.
I recreated the site with an updated theme, Parabola, and SLP is functioning properly.
Thank you for your assistance and the lesson in troubleshooting a WordPress site.

Issue:
Using Safari v. 10.0.1 or Firefox v. 50.1.0 on a Mac, when is zip code is entered that contains valid locations the locations and map will display. However, if you enter a zip code with no locations the map will not display. Then when you enter a zip code with locations, the locations will display but not the map. Use the following to reproduce the issue.Site: http://www.arizonahealthinsuranceexperts.com
1. Select “Locate An Agent” from the main menu
2. Enter 85012 – the locations and map will display as expected.
3. Enter 85259 – the locations and map will display as expected.
4. Enter 74014 – no locations will display which is correct. However, the map does not display.
5. Enter 85012 – the locations display but the map does not.When using Chrome v. 55.0.2883.87 on the Mac, the map is displayed properly in step #4 above. On a Windows desktop using Firefox, the plug-in functions properly.
Please let me know if you require additional information.
