I have upgraded to 4.94 version of SLP with no add-on. When a user enters their ZIP code it goes off and comes back with a blank screen still showing the Enter zip code box?
Could you provide your site URL and your plugin environment so we can take a look at what is happening with your site?

you may also find info in the troubeshooting or helpful hints. One issue may be that your Javascript in another plugin or theme is throwing errors preventing SLP from loading.
